Insurance Information
Although CBCC does not participate with any insurance companies, the services provided are often reimbursable through your plan. Many insurance plans offer out-of-network benefits. If you are interested in learning about what your plan covers, call the number on the back of your insurance card and ask what is covered for an out-of-network provider who is providing the services in which you are engaging (e.g. individual therapy, family therapy, or group therapy). Remember to ask if you have a deductible and what they consider the maximum allowable.
All services are billed directly to the client. Upon receiving payment you will be provided with a receipt that contains all of the necessary information that is required by insurance companies. You can submit this bill to the insurance company to be compensated through your out-of-network benefits.
Single Case Agreements (SCA)
Finding therapists who are trained in and implement adherent DBT can be incredibly difficult, and in-network providers are even more rare. The DBT-Linehan Board of Certification is deeply committed to providing resources to our clients to help them get insurance coverage for this life-saving care. Please follow the link to see instructions on how to request a “Single Case Agreement” from your insurance company, which may allow you to receive full reimbursement for your therapy.

Good Faith Estimate
Before beginning treatment, we will ask you whether you have health insurance or whether you plan to submit to your out-of-network benefits. If you do not have health insurance or do not plan to submit, we will provide you with a Good Faith Estimate to review and sign prior to beginning treatment. Due to the legal compliance required, we cannot begin treatment without this consent form signed and returned.
